Our diocesan safeguarding ‘policy and procedures’ document includes a set of forms for activities, recruitment/staff and reporting.
Apart from the GARDA VETTING FORM and the HSE REPORTING FORM, all the forms can be downloaded and amended as required to enter the specific parish contact information etc.
The HSE REPORTING FORM here is only intended as a sample. When making a report to the HSE you need to use the form available from your local HSE Child Social Workers.

Your safeguarding documentation was referred to at our meeting during the week and we were advised to look it up as it was so excellent. I am not sure how my name appeared on this page but as it has, may I say well done! I believe we have permission to use as template. Many thanks for that. We have policy etc in place but our very particular Chairperson wants to update it in line with new guidelines. As we struggled to know where to begin this is great news for us and will save time.
